If the sponsor is already in India: the affidavit shall be notarized by any notary public on stamp paper worth ₹200 — the tuition fee and special fee of the candidate for the entire course period will be borne by the sponsor (Annexure I). Proof of arrival and departure must be provided (copy of respective passport pages).

If the sponsor is in other countries: the sworn affidavit shall be attested by the Embassy or Consulate (Annexure I).

Payment Receipt

Application Fee ₹1,000/–. You can pay online or by RTGS, cheque or demand draft (DD). Make cheques or demand drafts payable to “Assisi College of Nursing” at Mukkoottuthara. The Cheque/Demand Draft should reach the office within seven days after the submission of the online application (address: The Principal, Assisi College of Nursing, Propose P.O., Erumely, Kottayam Dist. – 686509).

Important — the relationship must be clearly established. If the relationship is mentioned only as Niece / Cousin / Uncle / Aunt in the space provided on the certificate, the Village Officer must explain the exact relation in the certificate itself. Certificates or applications showing merely Niece / Cousin / Uncle / Aunt without a clear relationship link will be rejected.
