Before you start filling the online application form please make sure you have the following items ready at hand.

1. Copy of online payment receipt or scanned copy of demand draft of Rs.1000/ - in favour of Assisi College of Nursing, payable at Mukkoottuthara (Size should below 100kb in .jpeg format). Please refer the NRI prospectus for online payment details.

2. Proof of age and date of birth/SSLC/10th std. (Size should be below 200kb in .pdf format)

3. Certificate/Mark list of qualifying examination (Plus Two/12th std.) (Size should below 100kb in .jpeg format)

4. Scanned copy of signature of the candidate (Size should below 100 kb in .jpeg format)

5. Scanned copy of signature of the parent (Size should below 100 kb in .jpeg format)

After filling the application form

1. You will get a confirmation mail with printable version of your application form and further instructions. Ensure you have access to the mail id you are providing during application process.

2. The demand draft must reach the office within 7 working days after online registration, through registered post or speed post.
